Checklist for Your Appointment
To avoid delays, please bring the following.
Your valid passport (preferred), or one of the following:
Certificate of Identity
Refugee Travel Document
National Identity Card with photo (see INZ guidelines)
List of current medications, with names and dosages
Any relevant medical records or reports
Your glasses or contact lenses, if you use them
An interpreter, if you don’t speak English fluently
You’re welcome to bring a support person or family member
